OCR description
The covered entity (CE), Thomas Davies, DPM reported that its business associate (BA) experienced a hacking attack that compromised the protected health information (PHI) of 25,876 individuals. The PHI involved included demographic and clinical information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE and BA implemented additional administrative, technical, and security safeguards and offered free credit monitoring. OCR provided technical assistance regarding the HIPAA Rules.
