OCR description
The covered entity (CE), Blue Shield of California, reported that an employee mailed the protected health information (PHI) of 93,921 individuals to the wrong addresses. The PHI involved demographic information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals and implemented additional administrative, technical, and security safeguards to better protect its PHI.
