OCR description
Belkorp Ag, LLC, the covered entity (CE), reported that it experienced a hacking attack that compromised the protected health information (PHI) of 942 individuals. The PHI involved demographic, clinical, and financial information. The CE notified HHS and the affected individuals. In its mitigation efforts, the CE changed and strengthened password requirements. OCR provided technical assistance to the CE.
