OCR description
The business associate (BA), Blue Shield of California, reported that a coding issue resulted in the protected health information (PHI) of 607 individuals being mailed to the wrong individuals. The PHI involved names, dates of birth, and social security numbers. The BA notified HHS, affected individuals, and the media. In its mitigation efforts, the BA fixed the coding issue and provided the affected individuals with free credit monitoring. OCR provided technical assistance to the BA regarding the HIPAA Rules.
