OCR description
Vibra Hospital of Sacramento, LLC, the covered entity (CE), reported that several employees were the targets of an email phishing incident that affected the protected health information (PHI) of 620 individuals. The PHI included names, dates of birth, addresses, drivers’ license information, Social Security numbers, financial information, diagnoses and conditions, lab results, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional administrative and technical safeguards. OCR provided technical assistance regarding the HIPAA Breach Notification Rules. Staff were retrained.
