OCR description
The covered entity (CE), Houston Methodist Specialty Physician Group, reported that an employee mailed the protected health information of 2,329 individuals to the wrong recipients. The PHI involved included names and treatment information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE sanctioned the employee involved and retrained its workforce members to better protect its PHI.
