OCR description
The covered entity (CE), Yamhill Community Care Organization, reported that an excel sheet submitted to the Oregon Health Authority (OHA) was not properly redacted before OHA made it publicly available on their website, exposing the protected health information (PHI) of 1,251 individuals. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members.
