OCR description
The covered entity (CE), Bosch Choice Welfare Benefit Plan, reported that a vendor of its business associate (BA) experienced a cybersecurity incident that affected the protected health information (PHI) of 55,000 individuals. The PHI involved included names, Social Security numbers, dates of birth, claims and health insurance information, and diagnoses/conditions. The CE notified HHS and the BA notified the affected covered entities and the affected individuals. In response to the breach, the CE and BA implemented additional technical safeguards to better protect PHI.
