OCR description
The covered entity (CE), Synergy Advanced Healthcare LLC, reported that an employee impermissibly accessed and emailed the protected health information (PHI) of 1,260 individuals to her personal email account. The PHI involved included demographic information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
