OCR description
River Oaks Hospital & Clinics, the covered entity (CE), reported that two workforce members impermissibly accessed the protected health information (PHI) of approximately 2,742 individuals. The PHI included clinical, financial and demographic information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE sanctioned the responsible employees and implemented additional administrative and technical safeguards. Staff were retrained.
