OCR description
The business associate (BA), Optum Financial Services, reported that an employee of its subcontractor mailed the protected health information (PHI) of 2,124 individuals to the wrong individuals. The PHI involved clinical, demographic and financial information. The BA notified HHS, the affected individuals, and the media. In response to the breach, the BA’s subcontractor implemented additional technical and administrative safeguards and retrained the applicable workforce member to better protect PHI.
