OCR description

Delta Dental of Virginia, the covered entity (CE), reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 126,953 individuals. The CE notified HHS, affected individuals, and the media. In response to the incident and OCR’s investigation, the CE implemented additional technical safeguards.
