OCR description
The covered entity (CE), Sports Medicine & Orthopaedics, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 4,000 individuals. The PHI involved included clinical and demographic information. The CE notified HHS and the media and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
