OCR description
The covered entity (CE), Health First Health Plans, reported that its business associate (BA) experienced a ransomware incident that affected the protected health information (PHI) of 1,036 individuals. The PHI involved included names, addresses, dates of birth, and health insurance information. The CE notified HHS, affected individuals, the media. In response to the breach, the CE offered free credit monitoring to affected individuals and the BA implemented additional administrative, technical, and security safeguards. OCR also provided the CE with technical assistance.
