OCR description
Indian Health Board of Minneapolis, the covered entity (CE), reported that its business associate (BA) experienced a hacking incident that affected the protected health information (PHI) of 503 individuals. The PHI involved included demographic information. The CE notified HHS and the affected individuals. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals.
