OCR description
The covered entity (CE), Friends of Family Health Center, reported that a subcontractor of its business associate (BA) experienced unauthorized access to the protected health information (PHI) of 2,256 individuals. The PHI involved included demographic information. The CE notified HHS and the affected individuals. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals.
