OCR description
The covered entity (CE), Mission Neighborhood Health Center, reported that a subcontractor of its business associate (BA) experienced a hacking attack that affected the protected health information (PHI) of 3,471 individuals. The PHI involved included demographic information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE and BA implemented additional administrative, technical, and security safeguards to better protect PHI.
