OCR description
The covered entity (CE), Mendocino Community Health Clinic Inc., reported that a subcontractor of its business associate (BA) a cyberattack that compromised the protected health information (PHI) of 3,538 individuals. The PHI involved included financial and demographic information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals and the CE and BA and implemented additional administrative, technical, and security safeguards to better protect PHI.
