OCR description
The covered entity (CE), Exact Sciences Laboratories LLC, reported that an employee mailed the protected health information (PHI) of 2,658 individuals to the wrong recipients. The PHI involved included clinical and demographic information. The CE notified HHS, the media and affected individuals. In response to the breach, the CE implemented additional administrative and technical safeguards and retrained the employee to better protect PHI.
