OCR description
The covered entity (CE), Asian Americans for Community Involvement, reported that a subcontractor its business associate experienced a hacking incident that compromised the protected health information (PHI) of 521 individuals. The PHI involved demographic, clinical, and financial information. The CE notified HHS, affected individuals and the media. OCR provided technical assistance to the CE regarding the HIPAA Breach Notification Rule.
