OCR description
The covered entity (CE), Open Door Community Health Centers, reported that a subcontractor of its business associate (BA) experienced a cyberattack that compromised the protected health information (PHI) of 6,487 individuals. The PHI involved included financial and demographic information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals and the CE and BA implemented additional administrative, technical, and security safeguards to better protect PHI.
