OCR description
The covered entity (CE), Foundation Health Partners, reported that letters notifying patients that their provider was departing the CE’s clinic were sent to incorrect mailing addresses. The CE reported that the PHI included names for 523 individuals. Following the discovery of the incident, the CE provided notice to HHS, the affected individuals, and the media. The CE implemented additional administrative and technical safeguards to better protect its PHI.
