OCR description
The covered entity (CE), Iroquois Memorial Hospital reported that it had experienced a hacking attack that compromised the protected health information (PHI) of 621 individuals. The PHI involved demographic, clinical, and financial information. In response to the breach, the CE changed password, strengthened password requirements, implemented new technical and security safeguards, and provided individuals with free credit monitoring.
