OCR description
The covered entity (CE), Santa Cruz Community Health, reported that a subcontractor of its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 1,487 individuals. The PHI involved demographic, clinical, and financial information. The CE notified HHS, affected individuals, the media, and published substitute notice. The subcontractor notified law enforcement. The CE provided free credit monitoring to the affected individuals and has implemented new technical safeguards. OCR provided technical assistance regarding the HIPAA Breach Notification Rule.
