OCR description
The covered entity (CE), Native American Health Center, reported that its business associate (BA) experienced a hacking attack that affected the protected health information (PHI) of 2,451 individuals. The PHI involved included clinical and demographic information. In response to the breach, the BA provided complimentary credit monitoring services to the affected individuals, and implemented additional administrative, technical, and security safeguards to better protect PHI.
