OCR description
The covered entity (CE), AdventHealth Daytona Beach, reported that it experienced a loss of paper records during renovation that affected the protected health information of 821 individuals. The PHI involved included clinical, demographic, and financial information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE provided free credit monitoring to the affected individuals, and implemented additional physical safeguards to better protect its PHI.
