OCR description
The covered entity (CE), CashFlow Solutions, reported that certain employees were using personal devices and personal email accounts to transmit the protected health information (PHI) of 1,007 individuals. The PHI involved included clinical, demographic, and financial information. The CE notified HHS and the affected individuals. In response to the breach, the CE retrained workforce members and implemented new periodic technical and nontechnical evaluations to better protect its PHI.
