OCR description
The business associate (BA), Wakefield & Associates, LLC, reported that its third-party administrator experienced a hacking incident that affected the protected health information (PHI) of 31,751 individuals. The PHI involved demographic and financial information. The BA notified HHS, the affected individuals, and the media. The BA notified HHS, the affected individuals, and the media.
