OCR description
The business associate (BA), 180 Health Partners, Inc., reported that an employee was the subject of an email phishing attack that affected the protected health information (PHI) of 1,167 individuals. The PHI involved included clinical, financial and demographic information. The BA notified HHS, the affected individuals, and the media. In response to the breach, the BA implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
