OCR description
Health and Hospital Corporation of Marion County, the covered entity (CE), reported an employee email the protected health information (PHI) of 792 individuals to an unauthorized recipient. The PHI involved clinical and demographic information. The CE notified HHS, the affected individuals, and the media. In response to the breach, The CE sanctioned the employee and implemented additional technical and administrative safeguards.
