OCR description
Pafford Medical Services, the covered entity (CE), reported that its business associate (BA) experienced a hacking attack that affected the protected health information (PHI) of 1,000 individuals. The PHI involved included demographic information. In response to the breach, the BA implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
