OCR description
The business associate (BA), Mindoula Health, Inc., reported that a workforce member was the target of an email phishing attack that affected the protected health information (PHI) of 626 individuals. The PHI involved included clinical, demographic and financial information. The BA notified the affected covered entities, and on behalf of such entities, also notified HHS, and the affected individuals. In response to the breach, the BA implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
