OCR description
The covered entity (CE), Transitions of Western Illinois, Inc., reported that an employee left an unlocked shredding bin that contained the protected health information (PHI) of 805 individuals in an unsecured location. The PHI involved clinical and demographic information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE retrained its workforce and revised its PHI disposal procedures. OCR provided technical assistance to the CE regarding the HIPAA Rules.
