The covered entity (CE), First Choice Community Home Care, reported that its business associate (BA) experienced a breach of its network that affected approximately 725 individuals. The protected health information (PHI) involved demographic and clinical information. The CE notified HHS, affected individuals, and the media. OCR provided technical assistance to the CE regarding the HIPAA Rules.
