OCR description
The covered entity (CE), AccentCare, reported that business associate (BA) experienced a breach of its network that affected the protected health information (PHI) of approximately 19,101 individuals. The PHI involved demographic and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided the affected individuals with free credit monitoring. OCR provided technical assistance to the CE regarding the HIPAA Rules.
