OCR description
The covered entity (CE), Boston Mountain Rural Health Center, Inc., reported that its business associate (BA) experienced a breach of its network that affected the protected health information (PHI) of 4,800 individuals. The PHI involved included clinical and demographic information. The CE notified HHS, affected individuals, and the media. In response to the breach, the BA provided complimentary credit monitoring services to the affected individuals, and implemented additional administrative, technical, and security safeguards to better protect PHI.
