OCR description
The covered entity (CE), Greater Boston Urology, reported that several employees of its business associate (BA) were the targets of an email phishing scheme that affected the protected health information (PHI) of approximately 4,717 individuals. The protected health information (PHI) involved names, dates of birth, diagnoses/conditions, and other treatment information. The CE notified HHS and the affected individuals. OCR provided technical assistance to the CE regarding the HIPAA Rules.
