OCR description
The covered entity (CE), Community Nurse, reported that its business associate (BA) experienced a breach of its network that affected the protected health information (PHI) of approximately 6,746 individuals. The PHI involved demographic and treatment information. The CE notified HHS, affected individuals, and the media. OCR provided technical assistance to the CE regarding the HIPAA Rules.
