OCR description
The covered entity (CE), Mays Housecall Home Health, Inc., reported that it was notified by its business associate (BA) that it experienced a breach that affected the protected health information (PHI) 5,208 individuals. The PHI involved included clinical and demographic information. The CE notified HHS and the affected individuals. In response to the breach, the CE and BA implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
