OCR description
The covered entity (CE), Continental Casualty Company, reported that it was notified by its business associate (BA) that a subcontractor of the BA’s subcontractor experienced a breach of its network that affected the protected health information (PHI) of 6,086 individuals. The PHI involved included clinical, demographic, and financial information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE and BA implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
