OCR description
The covered entity (CE), Instabase, Inc., reported that it’s business associate (BA) experienced a breach of its network that affected the protected health information (PHI) of 908 individuals. The PHI involved demographic information. The CE notified HHS and the affected individuals. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals and the CE and BA implemented additional administrative, technical, and security safeguards to better protect PHI.
