OCR description
The covered entity (CE), Washington VA Medical Center, reported that an employee exposed the protected health information (PHI) of 1,467 individuals during the mail process. The PHI involved names, dates of birth, and social security numbers. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE is revising its relevant administrative processes. OCR provided technical assistance to the CE regarding the HIPAA Rules.
