OCR description
The covered entity (CE), Iowa Department of Health and Human Services, reported that it mistakenly posted a spreadsheet for a Medicaid waiver program on its website that contained the protected health information (PHI) of 6,717 individuals. The PHI involved health insurance information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE is revising its relevant administrative processes and retraining its workforce. OCR provided technical assistance to the CE regarding the HIPAA Rules.
