OCR description
The covered entity (CE), L.A. Care Health Plan, reported that an employee mailed the protected health information (PHI) of approximately 2,885 individuals to the wrong recipients. The PHI involved names, program types, and health plan names. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE is automating its report generating process. OCR provided technical assistance to the CE regarding the HIPAA Rules.
