OCR description
An unencrypted laptop computer containing the protected health information (PHI) of approximately 4,200 individuals was stolen from a personal vehicle. The PHI included names, addresses, phone numbers, dates of birth, social security numbers, treatment histories, and driver license numbers. The covered entity (CE) provided breach notification to the affected individuals, HHS, and the media. As a result of OCR's investigation the covered entity implemented new policies and procedures, retrained staff, and installed encryption software on all workstations.
