OCR description
The business associate (BA), DentaQuest, reported that an employee mistakenly posted a spreadsheet to its website that contained the protected health information (PHI) of 3,086 individuals. The PHI involved included clinical, demographic, and financial information. The BA notified HHS, the media, the affected individuals, and its covered entity (CE). In response to the breach, the CE and BA implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
