OCR description
The covered entity (CE), Wellmark Health Insurance, reported that an employee mailed the protected health information (PHI) of 6,666 individuals to the wrong recipients. The PHI involved included clinical information. The CE notified HHS, the media, and the affected individuals. In response to the breach, the CE retrained workforce members to better protect its PHI.
