OCR description
The covered entity (CE), Area Agency on Aging Pasco-Pinellas, Inc., reported that it was notified by its business associate (BA) that the BA experienced a breach involving improper storage/disposal of documents containing the protected health information (PHI) of 1,000 individuals. The PHI involved included demographic, clinical, and financial information. The CE notified HHS and the affected individuals. In response to the breach, the CE and BA implemented additional safeguards to better protect its PHI.
