OCR description
The covered entity (CE), Jason R. Egbert, OD, PC dba First Sight Family Vision, reported that it was notified by its business associate (BA) that it experienced a breach that affected the protected health information (PHI) of 1,225 individuals. The PHI involved clinical and demographic information. The CE notified HHS, the media, and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
