OCR description
The covered entity (CE), Kentucky MSO, LLC, reported that it was notified by its business associate (BA) that the BA experienced a breach of its network that affected the protected health information (PHI) of 537 individuals. The PHI involved included clinical and demographic information. The CE notified HHS, the media, and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
